Over the last decade, health insurance companies have started using AI algorithms (算法) to decide whether
they will pay for treatments your doctor recommends. This is different from the AI doctors use for diagnosis (诊断)
in that insurance AI focuses only on coverage and cost.
One of the most common examples is with prior authorization. Before your doctor can give you certain care,
they must get the insurer’s approval for payment. The AI checks if the treatment is considered medically necessary
and covered by your plan. It can limit the amount of care, like how many days you can stay in the hospital after an
operation. If the insurance company denies coverage for a treatment your doctor says you need, you’ll face tough
choices: appeal the decision (a slow process that rarely works), accept different treatment, or pay the very high cost
yourself.
Insurers argue these algorithms help make decisions quickly and safely, avoiding unnecessary treatments.
However, there is concern that they are often used to delay or deny effective care simply to save money. The systems
analyze patient records, but insurers keep exactly how the AI makes its decisions completely secret. This secrecy is
especially worrying because if an AI wrongly denies a claim very fast, the patient might get sicker or even die during
the long appeal process.
Unlike medical AI tools reviewed by government agencies, these algorithms are mostly unregulated. Their inner
workings are guarded as trade secrets. While some recent rules require insurers to consider individual patient needs,
companies can still largely set their own standards without outside checks.
Some places have proposed laws demanding doctor oversight of these algorithms. However, most proposed laws
still give insurers too much power to define medical necessity and don’t require independent testing for safety and
fairness before the AI is used. Experts argue government oversight is essential. The agency that reviews medical AI
could potentially take on this role. In the meantime, government regulators should require independent testing of
insurance AI for accuracy and fairness. Efforts to regulate this insurance AI are beginning, but much stronger action
is still needed.

Grinding (磨碎) rocks and spreading them over farmland may sound like a strange way to fight climate change,
but this approach, known as enhanced rock weathering, is gaining attention as a promising carbon removal strategy.
The technique speeds up a natural geological process and was recently highlighted in the XPrize Carbon Removal
competition. A UK-based startup called Undo received $5 million for applying this method to croplands in Canada.
In a trial, they removed 1,209 tons of carbon dioxide over a year.
Here’s how it works. Rainwater absorbs carbon dioxide from the air and becomes slightly acidic. When it falls
on certain rocks, it begins a chemical reaction that breaks down the rock. This process collection and carries it into
rivers and oceans, where it can be stored for thousands of years. Enhanced rock weathering works on a larger and
faster scale by crushing specific rocks into fine powder and spreading them across farmland. This increases the
surface area available for chemical reactions, pulling carbon dioxide from the atmosphere and storing it long-term.
The benefits extend beyond carbon removal. Farmers using this method may also see increased crop yields and
healthier soil. Rocks contain beneficial minerals like calcium, which can improve acidic soils, balance nutrients, and
reduce input costs for farmers.
However, there are challenges to applying this method. The needed minerals aren’t available everywhere, and
transporting crushed rocks can be costly and generate emissions (排放). Additionally, the energy and emissions
associated with mining, grinding, and transporting rock are concerns. Despite these issues, the scientific community
is optimistic. It’s estimated that billions of tons of carbon dioxide should be removed per year to meet climate targets.
