【Para.1】Cosmetic surgeons are increasingly concerned about the rise of “AI face”, as more and more clients arrive in their offices with unrealistic AI-generated visions of what they want to look like.

【Para.2】Dr Nora Nugent, a cosmetic surgeon from Tunbridge Wells, has seen this first hand. Clients have started coming to her office with photos of themselves beautified by AI and a false expectation that those results are achievable with surgery. She is also the president of the British Association of Aesthetic Plastic Surgeons, and says many colleagues are having similar experiences. “I can only predict an increase given the rate AI has been incorporated into every aspect of life,” she said.

【Para.3】People’s use of AI chatbots to come up with their ideal face means surgeons are increasingly presented with briefs for flawless skin, dramatically raised cheekbones, smooth noses and perfect symmetry that to achieve by means of surgery would be too time consuming, inordinately expensive – and often physically impossible.

【Para.4】AI can control every single pixel, but “surgery certainly doesn’t work on that microscopic detailed level. It just doesn’t,” said Dr Alex Karidis, a surgeon based in west London.

【Para.5】For many clients, however, those expectations are set in stone before they meet their surgeon. Karidis and Nugent describe how psychologically effective AI-generated images are in setting clients’ aesthetic goals. Nugent said: “Once you see an image, it’s wired into you.” Karidis agreed that AI images were “seared” into patients’ heads and said his colleagues had been bombarded with them recently.

【Para.6】The fact that the results of cosmetic surgery are not so easy to guarantee is something that surgeons are keen to make their clients aware of. “The patient has to understand that there is human variation in how they heal, how they age and what can be done as well,” said Nugent.

【Para.7】Yet AI agents give people a sense of control that does not exist in the real world. “I say to patients beforehand: It’s not limitless what I can do in surgery. Neither of us control everything.” To help understand the issue, I asked an AI agent to recommend me some cosmetic procedures and generated photos to show Dr Karidis. As I requested more and more changes to my appearance, the agent did start to warn me about the feasibility of such operations.

【Para.8】But Karidis says that when clients do their deep-dive research into surgeries, they tend to focus on the pictures, ignoring “all the noise around it”. “That’s the bottom line for everybody. The moment you show them something like that, that’s it.”

【Para.9】Surgeons have noticed certain consistencies in the style of AI-generated faces, in particular perfect symmetry, which is easy for an AI to achieve, but nearly impossible in real life. Say one of your eyes is a few millimetres higher than the other. It will take AI a few seconds to change, but according to Dr Julian De Silva, a cosmetic surgeon on London’s Harley Street, the rearrangement of pixels cannot be replicated in anatomy.

【Para.10】De Silva says that when AI is editing a photo of a client, it takes generally accepted parameters of beauty such as a V-shaped jawline for a woman, a sweeping “ogee curve” along the cheekbones, and a heart shaped face. For a man they often generate an abnormally wide jawline and low brows with fuller upper eyelids.

【Para.11】But there’s another aspect to the way AI is affecting cosmetic surgery which De Silva is concerned about: clinicians showing results from surgery on social media which seem incredible, but he believes are likely to be artificially generated. “I remember looking at one of these last week and I looked at it over and over,” says De Silva, recalling a video he saw in which a patient was made to look 30 years younger. “And then the 3rd time I watched it, I could see … the hands had six fingers.”

【Para.12】Having generated some beautified versions of myself, I got Dr Karidis’s thoughts on what the AI had recommended. In my AI inquiry, I said “I’m considering cosmetic surgery,” and requested “some enhancements to my photo”. I asked the chatbot to explain the virtual surgeries it had performed. The AI gave me rhinoplasty and septoplasty, “refining the nasal tip and straightening the bridge”. It also applied “a subtle blepharoplasty (eyelid lift) and brow refinement”.

【Para.13】Karidis said the rhinoplasty was “relatively modest” and the blepharoplasty “barely noticeable”, but that the work would cost “around £25,000”.
